HOME/HOSPITAL TUTORING
The purpose of tutoring is to enable a student to keep up with class assignments for enrolled courses during extended medical illness for a period non less than 14 days in the school year. The tutoring shall replicate the content of the student’s assigned classes. However, such a program shall be reduced if the child’s physician states that a lesser curriculum would be in the student’s best interest.
For the purpose of clarity, the responsibilities of parents and tutors are as follows:
PARENTS:
Obtain Physician’s Statement for Temporary Home or Hospital Education Form (28R/3), by contacting the Student Services at 508-885-8515. Return form signed by physician (medical doctor) to the Student Services Department at 302 Main Street, Spencer, MA 01562, or fax to 508-885-8541.
Spencer-East Brookfield Regional School District requires that an adult family member/responsible adult is present at all times while the tutoring takes place at the home.
Physician must complete the statement for Temporary home hospital education form in it’s entirety. Forms will be returned if the following information is not included:
The date the student was admitted to a hospital or confined to the home;
The medical reason for the confinement;
The expected duration of the confinement;
What medical needs of the student should be considered in planning the home or hospital services.
