SPENCER, MA - As of 2016, under the Affordable Care Act, all applicable large employers must show the IRS whether an offer of health insurance coverage was made by the employer to any employee scheduled to work 30 hours or more per week, and also what the monthly cost of that coverage would be for a single person at the lowest plan available. (It does not matter if you accepted an insurance with the district. This form is only to show the IRS that the district offered you health insurance coverage and what the cheapest plan offered by the district was.)
